Download and complete a Membership and Tournament Entry form and return to one of the follow locations no later than 5:00 p.m. on the Wednesday prior to the individual tournament date.

Eligibility
- Minimum of 18 years of age
- All players must have a current WHS/USGA handicap card
- Both male and female, amateur and professional players are eligible to complete
Fees

| Championship Entry | $110.00 | Tournament entry fee for 2-day championship (includes awards banquet) |
